Ministry staff failed to protect girl: report

Prince George -- Children's Ministry workers are partly to blame for the death of a four-year-old girl last fall, a report by the province's child protection director says.

Social workers made a "tragic unacceptable error in clinical judgment" by not acting on allegations of child abuse received the day before Amanda Simpson was hurt at home, Ross Dawson said in the report released yesterday.

A police investigation is continuing into Amanda's death. No autopsy report has been released and police have not commented on the child's injuries. CP
